Referrals will be made to Bury Housing Assessment by any agency between the hours of 9.00 am - 5.00 pm – Monday to Friday • If approached after 5.00 pm or at weekends, then agencies / individuals call the Homeless Out of Hours service. The Homeless out of duty officer will assess if the person is eligible to access the provision
Agency referrals only. Applications via gateway scheme, completed by referral agency prior to interview. Need proof of identity, eg birth certificate, national insurance number. Risk assessment from agency required.
Referrals can be emailed at any time. Assessments will be offered Monday to Friday between 10am and 4pm. Referral form can be requested via post or email. Applicants will ideally provide ID and proof of benefits (if available). References will be requested as part of the assessment process.
Phone Mon-Fri 9am-5pm for an application form, completed by applicant and referral agency prior to interview. Need proof of ID, eg letter from DWP with NI number or from other referral agency, and relevant reports.
