    Phone or call in Mon-Fri 9am-5pm or email. Application form filled in by all parties, assessed by a panel of cross-authority representatives. Evidence of serial exclusion, description of support needs and details of current and ongoing support network required. Panel will assess level of support needed and will inform applicant of outcome within 5 working days of monthly meeting. Interview for successful applicants with their agent, followed by risk/needs assessment.

Interview and assessment may be face to face, details may be taken by phone or referral form may be forwarded from eg Birmingham City Council Neighbourhood Office. New residents must make a claim for housing benefit on next working day. Initial referral of 28 days can be extended by 3 months subject to reassessment by Homeless Single Point of Access.
