Please send ID, proof of income and all supporting health and risk related documents with your application.
Referrals Mon-Fri 9am-5pm. Application form completed by referring agency prior to interview. Require proof of ID eg birth certificate, passport. Require risk assessment and relevant reports eg Probation report. Require proof of income.
Phone daily 9am-10pm or write, email or download form. Agencies must fill out a referral form. Self referrals can ask for help with filling out the application form and interview will be arranged at a later date. If no current vacancies, will be placed on a waiting list. Maximum number on the waiting list is six. Must be entitled to benefits.
