Referrals can be emailed at any time. Assessments will be offered Monday to Friday between 10am and 4pm. Referral form can be requested via post or email. Applicants will ideally provide ID and proof of benefits (if available). References will be requested as part of the assessment process.
The most common way to refer is via the Inform website. If it is a self-referral, or the referring agency does not have access to Inform, they can apply via the CHS website. Applications will be considered when a room becomes available. Suitable applicants will be invited for interview, and may bring the referrer/support worker or friend with them Offers will be made by the multi-agency Allocations Panel.
Phone, call in Mon-Fri 9am-5pm, or write for application form. Interview, with needs and risk assessments. Require proof of ID, eg birth certificate, passport, national insurance card, and any additional documentation.
Make an appointment by phoning or coming in to the centre between 8.30am and 5pm Monday to Friday. Applicants who are homeless on the day they attend must present before 3.30pm. please note due to covid 19 Housing Options are currently working remotely and contact should be made using 01904 554500
