Detailed referral with risk assessment (template available) in the first instance. If a potential suitable placement is identified, referee details are required, an interview with the young person will be carried out and then meet a greet with the potential host.
Phone Youthline (0300 303 0099, 24 hours daily) which is operated by St Basils in partnership with 4 other organisations. Youthline is responsible for assessing the support needs of young people aged 16-25 in Birmingham and for arranging appropriate levels of support.
Complete referral on Gateway through Housing Advice Centre or other professionals eg social workers. Require background information (previous housing history, substance use, mental health and offending issues) and proof of ID.
Make contact with LB Haringey Vulnerable Adults Team. Application form completed during meeting along with risk assessment and initial assessment. Require proof of ID eg NI number, passport, birth certificate. Additional information may be sought at initial support plan meeting.
