Phone Mon-Fri 9am-5pm. Application form. Interview. Require needs assessment, risk assessment, accommodation history and relevant reports. Also require proof of income and ID eg passport, birth certificate.
Referrals are all assessed at Northern Devon Hub, then passed to the appropriate accommodation provider. The referral will be put on a waiting list.A time is arranged for the prospective service user to visit the accommodation and have an interview with the service manager, referrer and/ or housing support worker. The service manager requires the Hub referral form before interview. Floating support -FS worker contacts the person referred and arranges either a home visit or a clinic appointment.
