Enquiries can be made at any time to the 24 hour helpline (0800 567 7699), however for referrals please call Monday to Friday between 9am – 5pm, if you are unable to do so you can call out of these hours and a scheduled call back will be booked to make a referral. Phone Mon-Sun 9am-9pm. Agency completes referral form and emails to referral maildrop box - All referrals to be typed not hand written. Referrer will be invited to attend an assessment along with who they have referred at the Lifehouse. The Assessment will then be discussed at the Adult Housing Panel to see if the referral is suitable for the Lifehouse. Pending outcome if successful the referred will have to wait for a bed to become available so they will be place on the waiting list.
Complete and submit a START application referral form and forward completed referral form to the Supporting People Commissioning team at Gloucestershire County Council. Referrals normally completed by P3 tel: 08081 786003, Homeless Team at Gloucester City Council 01452 396396 or Probation. All forms must be signed and dated by the applicant and immediately sent to the County Council for review before they are referred on to the appropriate supported housing provider.
