Single Homeless Team who is the gatekeeper to this service. The referrers come self referral, housing options, outreach team, Community Mental Health Team, Probation Service, etc.
Phone Mon-Fri 9am-5pm. Application form. Interview. Require needs assessment, risk assessment, accommodation history and relevant reports. Also require proof of income and ID eg passport, birth certificate.
Phone daily 9am-10pm or write, email or download form. Agencies must fill out a referral form. Self referrals can ask for help with filling out the application form and interview will be arranged at a later date. If no current vacancies, will be placed on a waiting list. Maximum number on the waiting list is six. Must be entitled to benefits.
