Phone daily 9am-8.30pm - may accept referrals later in emergency. Applicant/agency completes referral form prior to interview. Require proof of ID and background information about history of homelessness, offences, etc. If referring offenders or people with substance misuse issues a referring agency risk assessment may be required.
To make a referral you can telephone, drop in to our centre Monday-Friday 9am-5pm or email us. An application form must be completed by referral agency or the individual before interview. Risk assessment are always completed and we may require additional information to help us make our assessment. Referrals to supported houses via Initial Contact Team only.
1. Application form completed on paper or online - support can be provided to help complete if self referral
2. Risk assessment (if being completed by another referring agency)
3. Interview and assessment completed once applicant has met all eligibility criteria
