Interview and assessment may be face to face, details may be taken by phone or referral form may be forwarded from Birmingham Gateway supported housing access point. Beds allocated on first come first served basis.
Customer to undertake SIFA assessment will be completed and referral sent into service. Organisations can directly refer to Zambesi. Call and ask for referral application form. Customers can direct refer at service.
