Phone or call in Mon-Fri 9am-5pm or email. Application form filled in by all parties, assessed by a panel of cross-authority representatives. Evidence of serial exclusion, description of support needs and details of current and ongoing support network required. Panel will assess level of support needed and will inform applicant of outcome within 5 working days of monthly meeting. Interview for successful applicants with their agent, followed by risk/needs assessment.
