Phone or call in any time to request an application form. Submit an application form completed by prospective resident and/or any agency supporting them. The referral process includes an assessment of support needs and a risk assessment. The assessment will determine whether Riverside can provide the required support, in conjunction with outside agencies where appropriate. Proof of ID will need to be provided at the interview stage.
