All referrals must go via START (South Thames Assessment, Resource and Training Team) or Supported Living Team, the Complex Needs Panel.
Referrals will come through the First Contact Team and then be sent to the Lambeth Vulnerable Adults Pathway Manager who is based at Lambeth Assessment Centre.
The Vulnerable Adults and Mental Health Pathway act as gatekeepers and assess all potential referrals to see if they are appropriate. If accepted, are invited to visit project.
