Referrals are accepted directly and through other agencies. Referrals consist of a telephone interview, police check and a meeting before any decision is made.
Phone Mon-Fri 9am-4.30pm for application form. Interview.
Referrals will come through the First Contact Team and then be sent to the Lambeth Vulnerable Adults Pathway Manager who is based at Lambeth Assessment Centre.
Referrals accepted Mon-Fri 11am-5pm. Form completed whilst interview carried out by referral agency.
