To make a referral you can telephone, drop in to our centre Monday-Friday 9am-5pm or email us. An application form must be completed by referral agency or the individual before interview. Risk assessment are always completed and we may require additional information to help us make our assessment. Referrals to supported houses via Initial Contact Team only.
Phone or call in Mon-Fri 8.30am-6pm, or email. Application form completed by referrer and/or prospective resident with help of staff if required. If appropriate, this is followed by needs and risk assessment at face to face interview.
