Evidence of serial exclusion, description of support needs and details of current and ongoing support network required. Panel will assess level of support needed and will inform applicant of outcome within 5 working days of monthly meeting. Interview for successful applicants with their agent, followed by risk/needs assessment.
Call in at Midland Heart's Homeless Services Centre Mon-Weds, Fri 10am-6pm, Thurs 10am-2pm or phone daily 24 hours. Interview and assessment may be face to face, details may be taken by phone or referral form may be forwarded from eg Birmingham City Council Neighbourhood Office.
