Assessment undertaken by Northampton Borough Council or NAASH. Risk assessment. Need at least 1 form of ID if not known by the services.
Require proof of ID and income if possible.
Named agency referrals only through Inform. Assessments arranged at a time that suits all.

Must be able to prove local connection.
Basic needs and risk assessment. Additional information may sometimes be required.
Referrals come through the Lambeth Vulnerable Adults Pathway Manager.
