Assessment and reconnections toolkit

Tools for carrying out rapid comprehensive assessments and developing an effective offer for supported reconnection.

This toolkit aims to support voluntary and statutory agencies to develop a reconnection offer for people who are homeless and have a connection with another area.

Reconnection is the process by which people sleeping rough who have a connection to another area where they can access accommodation and/or social, family and support networks, are supported to return to this area in a planned way.

The toolkit includes the practical steps required to undertake reconnection, including identification, assessment, the verification of information and the types of reconnection options available. A template assessment form is included as a Word document that can be edited to meet the needs of your service.
