How to get a referral

Referral agencies

Only accept referrals from LB Hammersmith & Fulham Singles Team.

Referral procedure

Phone the LBHF Singles team, Mon-Fri 9am-5pm, to discuss referral eligibility and request a referral form.

Support offered

A team of 11 staff provide 24/7 support in a psychologically-informed environment. Personalised support provided around individual needs, through fortnightly keywork meetings and additional ad hoc support. Each resident develops an action plan alongside their key worker, to set agreed goals to work towards together, with an overall goal of moving on to more independent living.
