Rough Sleepers/ Single homeless people with support needs and a local connection to York. Can accept couples. Can accept people with dogs.

No automatic exclusions all referrals considered based on risk assessment

How to get a referral
Accept referrals via the Single Access Point from any professional agency. Most referrals come from York’s resettlement pathway from partner agencies.
Agency referrals only via single access point. Application forms from SAP@york.gov.uk or the website. Interview. Risk and needs assessment. Require proof of ID.
Length of stay
- Minimum stay: 3-6 Months
- Maximum stay: 2 Years
Support and accommodation
14 full time and 10 relief staff, 24 hour waking cover. Keywork system (5-6 residents per worker), regular house meetings. Action plans with agreed outcomes, reviewed at least every 3 months. Staff co-ordinate daily resettlement activities for residents through keywork, groupwork, multi-agency working and other activities. Resettlement programme

- Disabled facilities: 2 rooms fully accessible to wheelchair users. Lift to first floor rooms also.

York train station - 20 mins walk Project in the city centre so plenty of transport links
