Who is the project for?
Who do we help?
Single homeless people, particularly those sleeping rough, who have medium to high support needs around mental health, Mild Learning difficulties, substance misuse and/or offending behaviour. Applicants must have a connection to LB Hammersmith & Fulham.

How to get a referral
Referrals only accepted via LB Hammersmith & Fulham PATHS Team.
We only accept referrals via the local authority on King Street, Hammersmith & Fulham.
Length of stay
- Minimum stay: ? 1 Month
- Maximum stay: 6 months - 1 Year
Support and accommodation
13 staff, 24 hour waking hour cover. Keywork system with meetings as required. key workers assess residents' support needs and make referrals to relevant support agencies eg welfare benefits, meaningful occupation. Support plans with agreed outcomes, reviewed every two months.
