Referral agencies
Accept self referrals and referrals from any agency.
Referral procedure
Phone or call in 24/7 for application form, completed by applicant and referral agency.
Interview at service.
Risk assessment required. If a self-referral we can offer support to complete the form.

"24 hour cover. Allocated support worker, weekly meetings. Individual support/action plans, reviewed monthly.
Staff offer advice and support with budgeting skills, benefits, health issues, accessing leisure activities and finding employment or training.
Refer to external specialist agencies where appropriate.
